To make this change, you will have to fill out the Beneficiary designation form. If you have designated an irrevocable beneficiary, you will need to have this person's signature in order to make the change. The only exception to this is if your beneficiary was your spouse and you get divorced; the divorce automatically cancels this designation.
The form must be sent by mail to :
Desjardins Insurance
LEV 200 – 4 – B
200, rue des Commandeurs
Lévis (Québec) G6V 6R2
or by email to the Member Services.
The new beneficiary will appear on your next annual statement from the DGPP.
The Supplemental Pension Plans Act stipulates that upon the death of a member, the member’s spouse has priority over all other beneficiaries unless the
spouse has signed the form entitled
Spousal waiver of death benefits prior to retirement. This waiver may be revoked at any time by the spouse, prior to the death of the participant or before the commencement of the pension payments. This waiver only concerns benefits to be paid while the participant is still active in the plan. A new form must be signed at the participant's retirement in the event the spouse still wishes to waive the benefits he or she would be entitled to by virtue of his or her status of spouse after the participant's retirement.
Upon your death, the value of your annuity, if paid to your spouse, may be transferred tax-sheltered to his or her RRSP. This transfer in no way affects your spouse’s annual RRSP contributions or unused contribution room.
If the value of your annuity is paid to any other person, the amount is taxable. An adjustment may be necessary when this person produces his or her tax return.
